Bloodborne: The Old Hunters, première extension du jeu développé par FromSoftware et édité par Sony Computer Entertainment, est sortie le 24 novembre 2015 sur PlayStation 4. Classée dans les genres aventure et jeu de rôle, cette extension propose de nouveaux lieux, boss, armes et armures au sein d'un monde de cauchemar où les chasseurs du passé sont piégés à jamais. Le joueur explore des environnements inédits peuplés de créatures mortelles, découvre de nouveaux sorts et tenues, et croise de nouveaux personnages non jouables qui enrichissent l'histoire et le lore de Yharnam. L'intrigue plonge dans le cauchemar des premiers chasseurs, victimes des agissements du tout premier d'entre eux et condamnés à pourchasser sans fin des bêtes sous un monde déformé et sanglant.
D'un point de vue critique, l'accueil a été très favorable, avec une note moyenne agrégée de 90,90. Forbes a salué un ajout diaboliquement difficile à un jeu déjà brutal, transformant Bloodborne en une expérience encore plus monstrueuse et grotesque, tandis que DarkStation a estimé que l'extension venait confirmer la place du titre parmi les meilleurs de l'année. Du côté des joueurs, certains sur Metacritic n'ont formulé aucune plainte et ont jugé l'extension parfaite pour un jeu quasi parfait, allant jusqu'à la qualifier pour l'un d'eux de plus grande extension jamais réalisée, avec des notes maximales attribuées aux armes, boss, zones, histoire et personnages.

« Bloodborne : The Old Hunters is a massive DLC with incredible boss fights such as Lady Maria of the Astral Clocktower or Ludwig, the holy blade. The world in this dlc is massive and you can easily finish it in like 10 to 15 hours if you have a good level. The new weapons in the dlc are awesome and powerful.
