Ce titre en défilement latéral rend hommage aux premiers opus Sonic sur Sega Genesis : les joueurs incarnent Sonic, Tails ou Knuckles, chacun disposant de capacités propres, et traversent treize zones mêlant des niveaux revisités et de nouveaux environnements. Le jeu propose des stages spéciaux pour récupérer les Chaos Emeralds, un mode time attack avec classements en ligne et un multijoueur compétitif en écran splitté. L'intrigue suit Sonic et Tails enquêtant sur un signal énergétique à Angel Island, où les EggRobos de Dr. Eggman s'emparent de la Phantom Ruby et se transforment en Hard-Boiled Heavies, provoquant une dispersion des héros à travers des territoires connus tandis qu'Eggman cherche à exploiter le pouvoir de la gemme pour reprendre le contrôle de Little Planet. Disponible sur PC, PlayStation 4, Nintendo Switch et Xbox One, ce jeu fonctionne avec le moteur Retro Engine et affiche un temps de parcours principal d'environ cinq heures et demie, contre dix-neuf heures pour une approche complétiste. Commercialisé au prix de 19,99 euros, il obtient un score de 84 sur Metacritic et de 87 sur OpenCritic. La presse a salué l'œuvre, comme TheSixthAxis qui estime que Sonic Mania surpasse les originaux sur Mega Drive, ou Gaming Age qui y voit une entrée fantastique ayant ravivé l'intérêt pour la franchise. Certains joueurs, via Metacritic ou Steam, expriment leur enthousiasme en le qualifiant de meilleur Sonic 2D ou de jeu parfait, tout en regrettant parfois son incompletude sans le contenu téléchargeable.
