Condamnée à un état végétatif éternel, la très sexy Druuna attend d'être libérée d'un monde malade, claustrophobe et délabré, peuplé d'individus frappés par un terrible virus les transformant en créatures sanguinaires. Un ordinateur symbiotique directement relié au cerveau de Druuna constitue le seul moyen de capturer et de revivre les situations l'ayant plongée dans ce sommeil froid. Le joueur doit éviter les individus sans scrupules habitant l'esprit de la jeune femme et affronter les pièges disséminés sur le chemin incertain menant à la délivrance.
