Altered Beast dans sa version pour Sega Master System est paru le 1er juin 1989, portage développé par Sega R&D 2 et édité par Sega Enterprises, Ltd. ainsi que Tec Toy. Ce jeu de genre hack 'n' slash et beat 'em up présente des visuels fortement modifiés en raison des capacités techniques de la plateforme. À la différence d'autres versions, ce portage sur Sega Master System / Mark III ne propose qu'un mode joueur unique et ne comporte que quatre niveaux.
Le récit met en scène un ancien centurion romain rappelé de la mort par Zeus, le dieu du tonnerre, afin de sauver sa fille Athena des griffes de Neff, seigneur des enfers. D'après la description, le héros doit éliminer les cinq groupes d'hommes sous les ordres de Neff avant d'affronter le dieu, dans un périple de la surface jusqu'au cœur du monde souterrain peuplé de zombies, démons, gargouilles et loups à trois têtes. Le protagoniste reçoit le pouvoir de se transformer en bêtes magiques, passant de l'homme au superman, puis au loup-garou, dragon-garou, tigre-garou et enfin loup doré, chaque forme offrant des capacités surnaturelles pour vaincre un ennemi redoutable.
L'accueil critique de cette version a été relaté à travers divers avis. IGN qualifie Altered Beast de « relic of the arcade heyday that just doesn't hold up today », tandis qu'Official Xbox Magazine note qu'« it's fun for the first level, and that's about it. [Oct 2009, p.77] ». De leur côté, des joueurs sur Metacritic expriment des opinions contrastées, l'un déclarant « it's very old and Almost forgotten however Still fun and great the music are nice and gameplay are good.overall not bad experience », et un autre affirmant que « Altered Beast is a classic game that is stylish and creative. Not only that, but it doesn’t fail to deliver amazing gameplay. »
Pour un joueur visant à épuiser l'ensemble du contenu, le temps de completionist est estimé à environ une heure et demie.
