Ce jeu de course poursuit la série Road Rash avec un chapitre mêlant combat à moto et courses intenses, la trame principale demandant au joueur de faire évader Spaz de prison en intégrant un gang et en gravissant ses rangs par des défis et des victoires en course, tandis qu'un mode Five-O permet de jouer un policier chargé d'arrêter des suspects.
