Le jeu place le joueur aux commandes d'une équipe de la Major League Baseball ou de ligues mineures sélectionnées, offrant la possibilité de gérer une partie unique, une saison complète ou une franchise sur le long terme. Un mode carrière permet également de créer un athlète et de suivre son parcours professionnel.
L'accueil critique s'est révélé favorable, le titre affichant une note agrégée de 80,56. La presse spécialisée a salué la profondeur des ajouts pour les passionnés, Gaming Age estimant que les nouveautés « valent plus que le prix d'admission pour le fan hardcore », tandis que 1UP soulignait qu'il y en avait « pour tous ceux qui ont un lien avec ce sport ». Du côté des joueurs, un utilisateur de Metacritic a qualifié cet opus d'« étape emblématique » pour la franchise, avouant y avoir investi « de nombreuses heures » de jeu ininterrompu.

« For the hardcore fan, the new additions are more than worth the price of admission whether you are utilizing batting practice, adjusting your now expanded roster, or just enjoying new graphical touches such as post game highlight reels. « This was the iconic step up for the MLB The Show franchise and also the first I purchased and played non stop. Many hours were invested in this game. Despite some obvious flaws this I think will be the game that future franchise copies will be measured against. »
